Kenosha County Brookside Care Center Flooring Replacement

Background
The County of Kenosha, Wisconsin, through its Purchasing Division, is seeking bids to establish a contract for the replacement of existing carpet with vinyl plank flooring in various rehabilitation and patient rooms at the Brookside Care Center located at 3506 Washington Road, Kenosha, WI 53144.
The goal of this contract is to enhance the facility's interior by providing durable and aesthetically pleasing flooring options that meet the needs of its residents.

Work Details
The contractor will be responsible for the following tasks:
1. Remove existing carpet and vinyl floor base from designated areas.
2. Properly dispose of all debris from the removal process (County dumpsters cannot be used).
3. Prepare the concrete floor for new vinyl plank installation.
4. Furnish and install Shaw’s 5th & Main style Symbiotic 30 mil vinyl plank flooring in specified colors: 568 Sparrow for rehabilitation rooms and 7003 Cloves for other rooms.
5. Glue the vinyl plank to the concrete using manufacturer-approved adhesive.
6. Install new 6” Johnsonite vinyl cove base in Silk color.
7. Work will be phased; two rooms will be provided at a time for replacement activities, with a timeline for patching and painting walls between removal and installation phases.
8. The awarded contractor must supply product submittals for approval before commencing work.

Period of Performance
The contract will commence immediately upon award and will continue until project completion, estimated shortly after the bid due date of May 21, 2025.

All work will be performed at Brookside Care Center, located at 3506 Washington Road, Kenosha, WI 53144.

Bidder Requirements
Bidders must meet specific qualifications including: being an original manufacturer or authorized dealer of the flooring products; having provided similar services to comparable institutions within the last five years; completing a Vendor Responsibility Questionnaire; maintaining appropriate legal authority to operate in Wisconsin; providing evidence of financial capacity; and ensuring compliance with all insurance requirements as outlined in the solicitation.
