720-5161 Patient ID Wristband Sheets with labels

Background
The Department of Behavioral Health and Developmental Services (DBHDS) is issuing this Unsealed Request for Proposal (URFP) to establish a contract for the procurement of patient ID wristband sheets with labels and patient ID wallet cards. This contract will be utilized on an as-needed basis by DBHDS facilities throughout the Commonwealth of Virginia. DBHDS operates twelve facilities that provide intensive inpatient services, including psychiatric and psychosocial rehabilitation. The agency has identified a critical need for improved patient identification procedures to enhance care delivery and minimize errors.

Work Details
The contractor shall provide:

1. Patient ID Wristband Sheets with Labels:
- Sheet size: 8.5” x 11”, color: white, finish: matte.
- Each sheet must contain one wristband (size 1” x 8.375” to 10.5”) and four labels (small: 1” x 2”, large: 1” x 4”).
- Material must be soft yet durable, compatible with laser printers, resistant to water and hospital solvents, and designed for easy removal from the printed sheet.

2. Patient ID Wallet Cards:
- Sheet size: 8.5” x 11”, color: white, finish: matte.
- Each sheet must contain one wallet card (size: 3.375” x 2.125”).
- Material must be soft, durable, flexible, tear-resistant, compatible with laser printers, and allow for easy removal from the printed sheet.

3. Optional Supplies:
- Capability to furnish wristband clasps/locks that enhance resistance to removal by patients.

Period of Performance
The contract will have a period of one year from the date of execution by DBHDS and may be renewable based on special terms and conditions.

Place of Performance
The contract work will be performed at various DBHDS facilities throughout the Commonwealth of Virginia.
