Background
The Pennsylvania Department of Health is responsible for administering the Title V Maternal and Child Health Services Block Grant (MCHSBG) aimed at improving the health and well-being of mothers, infants, children, and youth, including those with special health care needs.

This Request for Applications (RFA) #67-188 seeks to address the needs of individuals living with sickle cell disease (SCD) through community-based services and support. The program aims to enhance access to care, promote client engagement, and develop public health partnerships, particularly for hard-to-reach populations.

Work Details
The Sickle Cell Community-Based Services and Support program will provide a range of supportive services including assistance with rent, utilities, transportation to appointments, and other necessary services.

Key deliverables include:
1) Grant Administration - appointing a Project Coordinator and Fiscal Officer;
2) Service Provider Identification - maintaining lists of supportive services;
3) Unified Care Plans for clients;
4) Client Assistance Fund administration;
5) Peer and Self-help Support networks;
6) Client Satisfaction Data Collection;
7) Health Disparities plan development.

The program will serve at least 80% children aged 0-21 years.

Period of Performance
The anticipated Grant Agreement term is from July 1, 2025 to June 30, 2028, subject to funding availability.

Place of Performance
Services will be provided across various regions in Pennsylvania as outlined in the RFA.
