Background
The State of Ohio, Department of Administrative Services, General Services Division, State Printing & Mail Services is seeking a contractor to provide a pamphlet for use by the Ohio Department of Job & Family Services. The purpose of this Invitation to Bid (ITB) is to obtain a contractor to provide this pamphlet for use by the Ohio Department of Job & Family Services.

Work Details
The contractor is required to provide a pamphlet in the following specifications: 8.5” X 11” size, 20 pages, printed two sides, head-to-head, black ink, stapled in the upper left corner. The stock should be 50# Uncoated White Offset. The pamphlets are to be labeled “JFS 07200-SPA (Rev. 6/24) Request for Cash, Food Stamp & Medical Assistance-Spanish” with total amount in carton not packaged quantity.
A pdf will be furnished to the selected vendor and all state supplied materials are to be returned to the proof person. No samples are available.
A proof shall be delivered to Family Assistance Attn: William Mercer at William.mercer@jfs.ohio.gov.

Period of Performance
The delivery is requested by August 1, 2024, or sooner. Invoices must reference job number SRC0000021780 with a signed delivery receipt.

Place of Performance
The delivery location is JFS Warehouse, 2098 Integrity Drive N., Columbus, OH 43209.
