9 by 12 Double Window Catalog Envelopes

Background
The State of Ohio, through its Department of Administrative Services, General Services Division, is issuing this Invitation to Bid (ITB) to procure 9 by 12 double window catalog envelopes for use by the Ohio Department of Taxation.
The goal of this contract is to ensure that the department has the necessary envelopes for its operational needs.

Work Details
The contractor will provide a total of 5,000 units of 9 x 12 blank double window envelopes with the following specifications:
- Size: 9 x 12
- Stock: 28# brown kraft
- Finished Trim Size: 9 x 12
- Packaging: The envelopes will be packaged in 10 boxes containing 500 envelopes each.
Cartons must be labeled with the total amount in each carton.
- An electronic proof must be delivered to Calvin Hughes at the specified address.
No samples are available, and a PDF of a previous proof will be provided to the selected vendor.
All state-supplied materials must be returned after use.

Period of Performance
The requested delivery date for the envelopes is March 28, 2025, or sooner.

Place of Performance
Delivery is to be made to Stephen Ortman at 4485 Northland Ridge Blvd., South Side Dock Door, Columbus, OH 43229.
