Livonia IRA - Kitchen Design

Background
The Finger Lakes Developmental Disabilities Service Office (DDSO) is seeking bids for the design of a kitchen renovation at the Livonia IRA, located at 6807 Richmond Mills Rd, Livonia, NY 14487.
The goal of this contract is to provide a professional kitchen design that meets the specifications required for the facility's operational needs.

Work Details
The vendor shall provide all labor and materials necessary to complete the kitchen renovation designs in accordance with the provided scope of work and architectural drawings.
The work includes:
1. Designs and specifications for Kitchen Replacement/Renovation;
2. Product Specification Data including construction details, material descriptions, dimensions, profiles, finishes, and applicable schematics;
3. All designs and specification documents must be submitted in Adobe PDF format;
4. Document submissions to OPWDD Point of Contact at routine intervals:
a. 90% Completion (Issued for Permitting - signed and stamped),
b. 100% Completion;
5. Internal Construction Permitting to be completed by the Maintenance Supervisor with necessary OPWDD Agency Personnel;
6. Provision of two sets of 24” x 36” blueprints upon completion, stamped/signed;
7. A Probable Construction Cost Estimate based on current pricing, materials, and supplies;
General Requirements include submission of Appendix A document and Certificate of Liability Insurance from the contractor after project award.

Period of Performance
The work is to be completed within 30 days after the award of the contract.

Place of Performance
The contract will be performed at Livonia IRA located at 6807 Richmond Mills Rd, Livonia, NY 14487.
