Printing and Delivery of Duplicate Title Forms (MVR-4)

Background
The North Carolina Department of Transportation (NCDOT), specifically the Division of Motor Vehicles (DMV), is issuing this Invitation for Bid (IFB) to seek qualified vendors to print and deliver Duplicate Title Forms (MVR-4).
The goal of this contract is to establish an Agency Specific Term contract for the provision of these forms.

Work Details
The vendor will be responsible for printing and delivering 150,000 Duplicate Title Forms (MVR-4).
The specifications for the forms include:
- Size: 8 ½ x 11 inches.
- Front: Black text with a copy void pantograph, high-resolution border, and micro printed interior border.
- Back: Text with an artificial watermark.
- Ink: Printed 2/2 with erasure sensitive ink, type MICR.
- Stock: Must use 24# Basic MOCR TF or 25# Basic MOCR TF security paper or functional equivalent with features including three family chemical sensations, invisible fluorescent fibers, toner retention, and reagents.
Delivery must be Free-On-Board (FOB) Destination to the NCDMV Warehouse located at 1529 S Blount St, Raleigh, NC 27603 within sixty (60) consecutive calendar days after receipt of order.
Forms should be packaged poly wrapped in quantities of 500 per pack and 2000 forms per carton.

Period of Performance
The initial term of the contract will be three (3) years from the date of final contract execution.
The state reserves the right to extend the contract term with vendor concurrence after the last active term.

Place of Performance
The contract will be performed at the NCDMV Warehouse located at 1529 S Blount St, Raleigh, NC 27603.

Bidder Requirements
Vendors must be registered as a North Carolina vendor in good standing and provide their eVP number.
Additionally, vendors are required to complete various attachments including HUB Supplemental Vendor Information, Customer Reference Form, Location of Workers Utilized by Vendor, Certification of Financial Condition, and Vendor Request for EO50 Price-Matching if applicable.
