Background
The Executive Office of Health and Human Services (EOHHS) is issuing this procurement to prepare Health Related Social Needs (HRSN) providers for the transition to managed care and promote the establishment and operationalization of Hubs.

The goal of the HRSN Integration Fund is to enter into Grant Agreements that will support Applicants in increasing knowledge and capacity, enhancing technology, and establishing systems and processes to effectively contract with MassHealth Accountable Care Organizations (ACOs) for HRSN Services, anticipated to begin on January 1, 2025.

EOHHS aims to distribute funding geographically throughout the Commonwealth, focusing on non-profit organizations that intend to contract with multiple ACOs.

Work Details
The HRSN Integration Fund will support various activities including:
- Technology enhancements for electronic referral, screening, case management, data management, financial management, and equipment for new staff.
- Development of business or operational practices to support HRSN Services delivery.
- Workforce development programs aimed at enhancing service delivery through cultural competency training and Community Health Worker certification.
- Outreach and education efforts aimed at improving health outcomes for Enrollees.

Eligible activities include: HRSN Supplemental Nutrition Services (e.g., medically tailored meals), housing support services (e.g., housing navigation), and specialized community support programs for homeless individuals or those with justice involvement.

Period of Performance
The Grant Agreements shall commence upon execution by the parties on an estimated contract start date of December 1, 2024, and shall terminate up to 24 months from the execution date.

Extensions may be granted up to June 30, 2027.

Place of Performance
The contract will be performed throughout the Commonwealth of Massachusetts.
