Crystal Lake Facility Renovation

Background
The County of McHenry is seeking to update vendor information as part of their procurement process. This initiative is in compliance with Illinois Public Act 102-0265, which mandates the collection of ownership status information for reporting purposes.

The goal is to ensure that all vendors are properly registered and that their information is accurate for future contracts and procurement activities.

Work Details
Vendors are required to complete a New/Update Vendor Information Form, which includes providing their Federal ID number or Social Security number, business name and address, remit address for payments (if different), and status of ownership.

Vendors must indicate if they are Minority-Owned, Women-Owned, Veteran-Owned, or a Small Business. Additionally, they need to certify whether they have obtained official certification from their state regarding their ownership status. The form also requires the primary contact person’s details for bids and purchase orders, as well as the names of persons authorized to sign bids, offers, or contracts.

Period of Performance
The vendor onboarding process must be completed by July 5, 2024.

Place of Performance
The services will be performed in McHenry County, Illinois.
