Family Caregiver Support Services

Background
San Bernardino County, through its Department of Aging and Adult Services - Public Guardian (DAAS-PG), is seeking proposals for the Title III-E Family Caregiver Support Program Services. This program aims to support informal, unpaid family caregivers who provide care to individuals aged 60 or older, individuals with Alzheimer's disease and related disorders, and older relatives caring for children under 18 or adults with disabilities.

The program is funded under the Older Americans Act (OAA) and aims to address the diverse needs of caregivers in the community.

Work Details
The contractor will provide a variety of services including: information services, counseling, training for caregivers, day care services, respite care, and outreach activities to ensure participation from eligible caregivers. Services must not replace existing funding sources but should supplement them.

The contractor is required to attend regular training meetings and ensure service locations are accessible to underserved populations. Additionally, a Management Information System (MIS) must be implemented to capture statistical and financial data related to service delivery.

Period of Performance
The contract will be awarded for a one-year period starting from July 1, 2025, through June 30, 2026, with options for three additional one-year renewals based on performance.

Place of Performance
Services will be provided within San Bernardino County, California.

Bidder Requirements
Proposers must demonstrate experience in providing caregiver support services and comply with all local, state, and federal regulations. They must also certify that they are not debarred or suspended from participation in federal programs and must be registered with the California Secretary of State if applicable.
