Background
The Los Angeles County Department of Public Health (LACDPH) is addressing the public health issue of Alzheimer's disease and related dementias, which affects an estimated 190,300 residents aged 65 and older in Los Angeles County. In response, LACDPH has initiated the Los Angeles County BOLD Initiative (LA BOLD), funded by the Centers for Disease Control and Prevention. This initiative includes the Healthy Brain LA (HBLA) Coalition, which aims to improve brain health and support individuals living with dementia and their caregivers.

The Coalition has developed a Strategic Plan for Alzheimer's Disease and Related Dementias for the years 2023-2028, which is now in the implementation phase.

Work Details
The selected vendor will be responsible for maintaining and engaging a Community Advisory Group (CAG) of 7-10 members. This includes planning and facilitating 2-3 meetings to support the Strategic Plan's implementation, organizing and conducting 1-3 interviews with CAG members or individuals with lived experience of dementia caregiving, and developing a summary report that synthesizes lessons learned from these activities. Specific tasks include:
1. Maintaining engagement with CAG members.
2. Planning and facilitating meetings with agendas developed in collaboration with LACDPH.
3. Conducting interviews to gather input for Coalition activities.
4. Developing a summary report based on meetings and interviews, ensuring all deliverables are reviewed by LACDPH prior to finalization.

Period of Performance
The contract will commence upon execution of the Purchase Order and will conclude on September 29, 2025.

Place of Performance
The services will be performed within Los Angeles County.
