24CS0061 Rehabilitation Services

Background
The California Department of Veterans Affairs (CalVet) is seeking qualified vendors to provide Temporary/Relief Rehabilitation Services, specifically Physical Therapist, Occupational Therapist, and Speech Language Pathologist services. The goal of this contract is to ensure that Veterans Home Residents receive high-quality rehabilitative care in compliance with federal and state regulations.

Work Details
The contractor shall provide all labor, materials, staff, licenses, permits, and other necessary items to perform rehabilitation services at the Veterans Home of California – Chula Vista. Services include:

1) Rehabilitative Services according to Medicare and Medi-Cal guidelines;
2) Physical Therapy, Occupational Therapy, and Speech Therapy services on a scheduled and as-needed basis;
3) Therapists must have at least one year of skilled nursing facility experience;
4) Weekly schedules for services must be provided in advance;
5) Documentation of progress in Resident medical records is required;
6) Training for restorative nursing aides and participation in care plan development meetings;
7) Billing will be based on hourly rates as specified in the Rate Sheet.

Period of Performance
The term of the agreement begins on December 1, 2024 or upon approval and ends on June 30, 2027.

Place of Performance
Services will be performed at the Veterans Home of California – Chula Vista located at 700 E Naples Ct. Chula Vista, CA 91911.
