Overview
Grant Description
The Health Federation of Philadelphia proposes to have 119 AmeriCorps members who will provide individuals in underserved communities with health access and education throughout Pennsylvania, Illinois, Florida, California, and New York. At the end of the first program year, the AmeriCorps members will be responsible for providing 120,000 underserved individuals with health access and education.
In addition, the AmeriCorps members will leverage 1000 volunteers who will be engaged in providing health access and education in underserved communities. This program will focus on the CNCS priority area of healthy futures, including addressing the opioid epidemic, increasing seniors' ability to remain in their own homes with the same or improved quality of life for as long as possible, and increasing physical activity and improving nutrition with the purpose of reducing obesity.
The CNCS investment of $1,653,267 will be matched with $1.6 million in public and private funding.
