Tribal Self-Governance Planning Cooperative Agreement Program

ID: HHS-2025-IHS-TSGP-0001 • Type: Posted

Description

Posted: Dec. 18, 2024, 12:00 a.m. EST
The purpose of this Planning Cooperative Agreement is to provide resources to Tribes interested in entering the Tribal Self-Governance Program (TSGP) and to existing Self-Governance Tribes interested in assuming new or expanded Programs, Services, Functions, and Activities (PSFA). Title V of the Indian Self-Determination and Education Assistance Act requires a Tribe or Tribal organization (T/TO) to complete a planning phase to the satisfaction of the Tribe. The planning phase must include legal and budgetary research and internal Tribal government planning and organizational preparation relating to the administration of health care programs. See 25 U.S.C. 5383(d). The planning phase is critical to negotiations and helps Tribes make informed decisions about which PSFAs to assume and what organizational changes or modifications are necessary to successfully support those PSFAs. A thorough planning phase improves timeliness and efficient negotiations and ensures that the Tribe is fully prepared to assume the transfer of IHS PSFAs to the Tribal health program.

Background
The Tribal Self-Governance Planning Cooperative Agreement Program is administered by the Indian Health Service (IHS) under the Division of Grants Management. The program aims to provide resources to Tribes interested in entering the Tribal Self-Governance Program (TSGP) and to existing Self-Governance Tribes looking to expand their Programs, Services, Functions, and Activities (PSFAs). This initiative is rooted in Title V of the Indian Self-Determination and Education Assistance Act, which mandates a planning phase that includes legal and budgetary research as well as internal Tribal government planning for health care program administration. The goal is to enhance Tribes' ability to manage health care programs tailored to their community needs.

Grant Details
The Planning Cooperative Agreement provides resources for Tribes and Tribal Organizations (T/TO) interested in entering the TSGP or expanding their program offerings. The TSGP allows Tribes to negotiate with IHS to assume PSFAs, granting them authority over health care management. The agreement requires a comprehensive planning phase that aids Tribes in making informed decisions regarding which PSFAs to assume and what organizational changes are necessary. Key tasks include: conducting legal and budgetary research, internal planning, understanding funding distributions, identifying PSFAs for assumption or expansion, evaluating current health service levels, and preparing for negotiations with IHS.
