The purpose of this program is to provide diabetes treatment and/or prevention activities and/or services (also referred to as activities/services ) for American Indian / Alaska Native (AI/AN) communities. Awardees will implement one Special Diabetes Program for Indians (SDPI) Diabetes Best Practice (also referred to as Best Practice ) and report data on the Best Practice's Required Key Measure (RKM). Awardees may also implement other activities/services based on diabetes-related community needs and develop an evaluation plan. Activities/services will be aimed at reducing the risk of diabetes in at-risk individuals, providing high quality care to those with diagnosed diabetes, and/or reducing the complications of diabetes.

Background
The Special Diabetes Program for Indians (SDPI) is a program authorized under the Snyder Act, the Transfer Act, and Section 330C of the Public Health Service Act. It aims to address the diabetes epidemic among American Indian/Alaska Native (AI/AN) people, who are disproportionately affected by diabetes.

Grant Details
Awardees are required to provide diabetes treatment and/or prevention activities and services for AI/AN communities. They must implement one SDPI Diabetes Best Practice and report data on the Best Practice’s Required Key Measure (RKM). Awardees may also implement other activities/services based on diabetes-related community needs and develop an evaluation plan. The total funding identified for fiscal year (FY) 2023 is approximately $136 million, with individual award amounts anticipated to be between $12,500 and $7.5 million. The period of performance is for 5 years. The Indian Health Service (IHS) will have substantial programmatic involvement in the project during the entire period of performance.

Eligibility Requirements
To be eligible for this funding opportunity, applicants must be an Indian health program operated by an Indian Tribe or Tribal organization pursuant to a contract, grant, cooperative agreement, or compact with the Indian Health Service pursuant to the Indian Self-Determination and Education Assistance Act; an urban Indian health program operated by an urban Indian Organization pursuant to a grant or contract with the Indian Health Service pursuant to title V of the Indian Health Care Improvement Act; or the Indian Health Service.

Period of Performance
The period of performance is for 5 years.
