The Consolidated Appropriations Act, 2021 (P.L. 116-260), which contained the Coronavirus Response and Relief Supplemental Appropriations Act, 2021 (P.L. 116-260, Section 2, Division M) provided, in part, funding for strategies to improve testing capabilities and other COVID-19 response purposes in populations at high-risk and underserved, including racial and ethnic minority groups and people living in rural communities. Strategies also include those to develop or identify best practices for states and public health officials to use for contact tracing. To achieve these purposes, the Centers for Disease Control and Prevention (CDC) is announcing a non-competitive grant CDC-RFA-OT21-2103 titled National Initiative to Address COVID-19 Health Disparities Among Populations at High-Risk and Underserved, Including Racial and Ethnic Minority Populations and Rural Communities. This grant will provide funding to state, local, US territorial and freely associated state health departments (or their bona fide agents) to address COVID-19-related health disparities and advance health equity by expanding state, local, US territorial and freely associated state health departments capacity and services to prevent and control COVID-19 infection (or transmission).
Background
The Consolidated Appropriations Act, 2021 provided funding for strategies to improve testing capabilities and other COVID-19 response purposes in populations at high-risk and underserved, including racial and ethnic minority groups and people living in rural communities. The grant aims to address COVID-19-related health disparities and advance health equity by expanding state, local, US territorial and freely associated state health departments capacity and services to prevent and control COVID-19 infection among these populations.

Grant Details
The grant will be used to strengthen public health infrastructure, preparedness, and response capabilities in state, local, US territorial, and freely associated state health departments to address COVID-19 related health disparities. It will focus on expanding existing and/or developing new mitigation and prevention resources and services, increasing/improving data collection and reporting for populations experiencing a disproportionate burden of COVID-19 infection.

It will also focus on building, leveraging, and expanding infrastructure support for COVID-19 prevention and control among populations that are at higher risk and underserved. Additionally, the grant aims at mobilizing partners and collaborators to advance health equity and address social determinants of health as they relate to COVID-19 health disparities among populations at higher risk and that are underserved.

Eligibility Requirements
The grant is open for competition. Eligible applicants include state, local, US territorial, and freely associated state health departments (or their bona fide agents).

Period of Performance
The total performance period is two years with all funding disbursed during year one. The estimated award date is June 01, 2021.

Grant Value
$2,250,000,000 is the total period of performance funding with an average one-year award amount varying by jurisdiction category.

Place of Performance
The grant will be performed in state, local, US territorial, and freely associated state health jurisdictions.
