Background
The Fiscal Year 2025 Health Center Controlled Network (HCCN) Cooperative Agreements are issued by the Health Resources and Services Administration (HRSA), specifically through the Bureau of Primary Health Care. The mission of this grant is to support Health Center Controlled Networks (HCCNs) in leveraging health IT and data to enhance the delivery of affordable, accessible, and high-quality primary health care.

The focus areas include data management and analytics, interoperability of systems and digital health tools, and the implementation of the Uniform Data System Modernization (UDS+). HCCNs have been providing essential health IT support for over 20 years, with a significant percentage of health centers partnering with them to improve care access and quality.

Grant Details
The HCCN funding aims to support networks of participating health centers (PHCs) in enhancing their operational capabilities through specific tasks:
1) Improve clinical quality, health outcomes, and operations through data management and analytics;
2) Enhance interoperability with healthcare providers by integrating external data sources into electronic health records;
3) Facilitate UDS+ implementation by ensuring PHCs submit disaggregated patient-level data as required by HRSA.

Award recipients will work on three required objectives and two elective objectives tailored to their network's needs. They must also develop individualized work plans with each PHC within 90 days of the award start date.

Eligibility Requirements
Eligible applicants include Health Center Controlled Networks or current Health Center Program (H80) award recipients that have been funded for at least two consecutive years. Organizations must have at least ten PHCs in their network, be majority-controlled by H80 award recipients, and maintain an independent governing body.

Eligible organizations can include public or private institutions of higher education, non-profits, for-profit organizations, state or local governments, and Native American tribal governments or organizations. Individuals are not eligible to apply.

Period of Performance
The period of performance for this grant spans three years from August 1, 2025, to July 31, 2028. Funding will be allocated in three budget periods of twelve months each.

Grant Value
The total available funding for this opportunity is $48 million, with individual awards ranging from $705,000 to $2.955 million per budget period depending on the number of PHCs in the applicant's network.

Place of Performance
The grant activities will be performed across various locations where participating health centers are situated within the United States.

A Health Center Controlled Network, or A current Health Center Program (H80) award recipient, funded for at least two consecutive preceding years, applying on behalf of an HCCN. To be an HCCN, you must meet the following criteria throughout the period of performance: Have at least 10 PHCs, Be majority-controlled and, as applicable, at least majority-owned by Health Center Program (H80) award recipients, and Have your own governing body independent of the boards of your health center members.
