Background
The U.S. Department of Health and Human Services is offering funding under the fiscal year (FY) 2025 Expanded Hours (EH) program. The EH funding aims to expand access to health center services by increasing health center operating hours to meet identified patient and community needs. The Health Center Program is authorized by 42 U.S.C. § 254b (Title III, § 330 of the Public Health Service (PHS) Act).

Grant Details
The EH funding will expand access to health center services by increasing health center operating hours to meet identified patient and community needs. This includes expanding hours for one or more in-scope service delivery sites, providing in-person appointments during the expanded hours, and maintaining compliance with all Health Center Program requirements and applicable laws and regulations. Recipients must document use of EH funds separately and distinctly from other Health Center Program funds and other federal award funds.

Eligibility Requirements
Current Health Center Program operational (H80) grant award recipients are eligible to apply for the grant. Cost sharing or matching is not required for this program. However, applications requesting more than $500,000 annually or failing to include a Project Narrative may be considered ineligible for review.

Period of Performance
The period of performance for the grant is from December 1, 2024, through November 30, 2026 (2 years). Recipients must begin to increase operating hours at one or more proposed sites within 6 months of award and document the increase through a Form 5B Scope Adjustment Request in EHBs. Recipients must also implement their entire proposed increase in hours of operation within 18 months of award.

Grant Value
The anticipated total available funding for FY 2025 is $60 million, with approximately 120 new grants expected to be awarded. Recipients may apply for up to $500,000 per year, with the potential to request up to $150,000 of the $500,000 in year 1 for minor alteration/renovation and equipment costs.
