Sources Sought Notice for Ambulance Service at VAMC Cincinnati | Seeking qualified Small Business prospective offerors with OMTB certification to determine set-aside type for the forthcoming solicitation The purpose of this sources sought notice is to seek responses from all qualified Small Business prospective offerors to include SDVOSB/VOSB verified vendors who hold a current Ohio Medical Transportation Board (OMTB) certificate to perform ambulance service in the state of Ohio to determine the type of set-aide that will be utilized for the forthcoming solicitation, summarized as follows: Short Title: Ambulance Service (Advanced and Basic Life Support) for VAMC Cincinnati NAICS-621920: Ambulance Services Purpose: The purpose of this sources sought notice is to determine what type of set-aside is appropriate for this acquisition Place of Performance: Department of Veterans Affairs | Cincinnati VA Medical Center (VAMC), 3200 Vine Street, Cincinnati, OH 45220 Anticipated Period of Performance: 22 January 2019 to 21 January 2024 (Base Year plus Four Option Years) General Requirements: The contractor shall provide all vehicles, personnel, management, transportation, materials, supplies, and equipment necessary to perform contract services as defined and specified in the PWS which shall be provided in the forthcoming solicitation. The contractor shall perform services under this contract in strict adherence to all current laws and regulations applicable to the state of Ohio unless supplemented to exceed those requirements as specified in the PWS. All certification, equipment, staffing and recordkeeping should be in compliance with the rules of the Ohio Medical Transportation Board (formerly the Ohio Ambulance Licensing Board) and appropriate parts of Ohio Administrative Code 4766-3 for ambulette service.
