Background
The Dzilth-Na-O-Dith-Hle Health Center, located in Bloomfield, NM, is seeking to contract for non-personal healthcare services. The goal of this contract is to provide one (1) Outpatient Department (OPD) Nurse to support the health center's mission of delivering quality healthcare services to the community.

Work Details
The contractor shall provide non-personal health care services for one (1) Registered Nurse for a total of 1040 hours. Services must be performed in accordance with the Performance Work Statement (PWS) outlined in Attachment A. This is a Labor-Hour Contract Line Item Number (CLIN), and values are estimated; the contractor assumes risk if exceeding these values. The contract includes an option period for an additional 1040 hours of nursing services, also subject to the same terms as the base period.

Period of Performance
The initial period of performance is from April 6, 2025, to April 5, 2026, with an option period extending from October 6, 2025, to April 5, 2026.

Place of Performance
The services will be performed at the Dzilth-Na-O-Dith-Hle Health Center located at 6 Road 7586, Bloomfield, NM 87413.
