Relocate and Install two (2) existing Elko Mobile Units   4

This is a SOLICITATION NOTICE for the upcoming Solicitation 75H701-24-R-00027 Relocate and Install two (2) existing Elko Mobile Units, located in Elko, NV. These construction services will be in support of the Indian Health Service (IHS) prepared in accordance with FAR Part 36.

SCOPE OF WORK: The purpose of this project is to relocate and install two (2) existing Mobile Units at the Southern Bands Health Center (SBHC). Work consists of transporting Mobile Units from on-site temporary staging area to permanent locations, site development and installation, and connection to existing electric, telecommunications, water and sewer systems utilities.

LOCATION OF PROJECT: All work shall be performed at the Southern Bands Health Center, which is located at 515 Shoshone Circle, Elko, Nevada 89801.

CONSTRUCTION DURATION: Period of Performance is 150 calendar days from issuance of the Notice to Proceed.

CONSTRUCTION MAGNITUDE: The magnitude of this construction project is anticipated to be between $500,000 and $1,000,000, in accordance with FAR 36.204.

SITE VISIT: A site visit will be held on June 4, 2024 at 11:00 AM PDT (local Time). Please confirm attendance in accordance with clause L.6. FAR 52.236-27 Site Visit (Construction) (Feb 1995) Alternate I (Feb 1995).

CONTRACT TYPE: The Indian Health Service intends to award a single Firm Fixed Price (FFP) contract in support of this requirement.

This solicitation will be procured as a Request for Proposal (RFP) in accordance with FAR Part 36 procedures with Lowest Price Technically Acceptable (LPTA) evaluation criteria.

This project will be solicited as a 100% Small Business set-aside under North American Industry Classification System (NAICS) code 236220 Commercial and Institutional Building Construction. The associated size standard for this procurement is $45 million.

For the purposes of this procurement, a concern is considered a small business if its average annual revenue for the last five (5) years is less than $45 million. For information concerning NAICS and SBA size standards, go to http://www.sba.gov.

Registration in the System for Award Management (SAM) database is mandatory to be considered for award. Offerors may obtain information on registration and annual confirmation requirements via the internet at: https://sam.gov. Contractors are encouraged to complete SAM registration as soon as possible; the Government may not delay award for the purpose of allowing a contractor additional time to register in SAM.

Amendment A00001 issued 5/20/2024 to provide responses to Pre-Proposal Inquiries 1 to 3, make revisions to clauses H.8 Key Personnel and L.11. General Instructions and to replace Attachment J06 PPQ.

Amendment A00002 issued 6/7/2024 to provide a copy of the Site Visit Agenda/Minutes and Sign-in Sheet, to remove Attachments J04 and J04a and to incorporate Section H, I and K clause changes.

Amendment A00003 issued 6/20/2024 to provide responses to pre-proposal inquiries 4-8, extend the proposal due date to July 2, 2024, and replace Attachment J01d Elko Mobile Units Foundation System.

Amendment A00004 issued 6/21/2024 to include the responses to pre-proposal inquiries 9-13 and to extend the proposal due date FROM July 2, 2024 TO July 26, 2024 by 2:00pm MST. The extension is due to a forthcoming scope addition currently in development and estimated to be provided via amendment on or around July 15, 2024.

Amendment A00005 issued 6/29/2024 to correct the proposal due date time zone from MST to PDT. The proposal due date is July 26, 2024 by 2:00 PM PDT. Included in this modification is an updated SF1442, updating Block 13.

Amendment A00006 issued 7/5/2024 to replace the wage determination with the attached revised Attachment J02 Wage Determination: General Decision Number NV20240035 Mod 4 dated July 5, 2024. The proposal due date of July 26, 2024 by 2:00 PM PDT remains unchanged.

Amendment A00007 issued 7/19/2024 to extend the proposal due date from July 26, 2024 to August 2, 2024 by 2:00 PM PDT. Included in this modification is an updated SF1442, updating Block 13.

Amendment A00008 issued 7/27/2024 to extend the proposal due date from August 2, 2024 to August 12, 2024 by 2:00 PM PDT. Included in this modification is an updated SF1442, updating Block 13.

Amendment A00009 issued 7/29/2024 to add an RV parking pad to the project and to replace Attachments J01 SOW - Elko Mobile Units, J01c Elko Mobile Units Reference Drawings, and J01d Elko Mobile Units Foundation System. The proposal due date of August 12, 2024 by 2:00 PM PDT remains unchanged.

Amendment A00010 issued 7/29/2024 to provide a response to pre-proposal inquiry #14: Question #14: Is the range still $500K -1M with all the changes to the project? Answer # 14: The magnitude of this construction project remains unchanged and is anticipated to be between $500,000 and $1,000,000. The proposal due date of August 12, 2024 by 2:00 PM PDT remains unchanged.
