This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for quotes being requested. The solicitation is issued as Request for Quote (RFQ) # 36C25623Q1573. Contractor shall provide onsite Physician Anesthesiologist and Certified Registered Nurse Anesthetists (CRNAs) services for the Michael E. DeBakey Veterans Affairs Medical Center Houston, TX.

Background
This is a solicitation for anesthesiology services at the Michael E. DeBakey VA Medical Center (MEDVAMC) in Houston, TX. The contract is issued by the Department of Veterans Affairs and is a Service Disabled Veteran Owned Small Business (SDVOSB) set aside. Offerors must be VetCert certified at the time of quote submission.

Work Details
The contract requires the provision of onsite Physician Anesthesiologist and Certified Registered Nurse Anesthetists (CRNAs) services at MEDVAMC. The work includes preoperative assessment, intraoperative anesthetic administration and monitoring, post-anesthesia care, and emergency airway and resuscitation management throughout the facility. The contractor is responsible for providing a point of contact for coordination and reporting problems 7 days a week, 24 hours a day.

Period of Performance
The contract period includes a base year with an option for four additional years. The performance work statement specifies that the contract may be extended for up to 6 months.

The services are to be performed onsite at the Michael E. DeBakey VA Medical Center (MEDVAMC) located at 2002 Holcombe Boulevard, Houston, TX 77030.
