SOURCES SOUGHT SHOULDER SPECIALIST The Department of Veterans Affairs, VISN-20 VA Healthcare System is requesting information related to vendors capable of providing Orthopedic Surgeons (Shoulder Specialist). The Services will be conducted at the Boise VA Medical Center in Boise Idaho. This is not a request for bids, quotes, or proposals. This is a SOURCES SOUGHT announcement only. The purpose of this notice is to conduct market research and obtain information from qualified businesses. The responses to this announcement will assist in determining the socio-economic set aside if feasible. If an insufficient amount of small business responses are received to establish the basis for setting this action aside for other than large business participation then the action will be advertised as full and open competition, in which case, all interested parties responding shall be eligible to bid. This notice shall not be construed as a commitment by the Government to ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. The vendors are requested to respond to the requirements below and submit related supporting information/documentation. PROOF OF PROVIDER QUALIFICATIONS: The Contractor shall provide resumes demonstrating that the intended providers fulfill the following requirements: a. Trainees/fellows in the program must be board-certified or board-eligible orthopedic surgeons, and be under the tutelage of a board-certified, fellowship-trained orthopedic shoulder sub-specialist. b. Have an unrestricted license in a US state, territory or US commonwealth; c. Speak, read and write English proficiently; d. Provide evidence of demonstrated current clinical competence to perform appropriate care services; e. Have a professional degree and education applicable to medical staff members, e.g., MD, DO; f. Provide evidence of current professional liability insurance; g. Have current ACLS certification h. Providers will need to be VetPro Certified; and i.
