STATEMENT OF WORK FOR
6th Medical Group Medical Waste
2 May 2022
Place of Performance: 6th Medical Group, 3250 Zemke Ave, MacDill AFB FL 33621 & Sabal Park Clinic, 9208 King Palm Drive, Tampa FL 33619.
1. Description of Service: Contractor will provide all labor, tools, supplies, and equipment to pick-up and dispose MacDill & Brandon Clinic Medical Waste. Service shall be provided once a month for MacDill and four times per month for Sabal Park Clinic. The contractor shall be responsible for the following:
1.1. Provide all container for MacDill that holds up to 4000 lbs of red bag/sharps waste, must be in a locked container; and two 97-gallon containers on wheels for Sabal Park Clinic.
1.2. Waste pick-up from the MacDill Clinic by the fifth workday of each month.
1.3. First pickup from the Sabal Park Clinic by fifth workday of the month and three other pick-up thereafter for the month no longer than seven days apart.
1.4. Pick-up times from the MacDill are between 0730-1630, Mon-Fri, excluding Federal Holidays, and Down days.
1.5. Pick-up times from the Sabal Park clinic are between 0800-1700, Mon-Fri, excluding Federal Holidays and Down Days.
1.6. Provide copy of manifest along with the Certification of Destruction and a copy of condition/guidelines for managing infectious/medical waste.
1.7. Ensure the container has the EPA approve biohazard symbol on the outside of the container and the BIOHAZARD
1.8. Collect and transport medical waste via sealed truck to incineration site designate by the contractor.
1.9. Ensure destruction of disposal procedure is based on the state, county, and federal regulations.
1.10. Sign the manifest prior to waste collection; one copy will be left each generator (Facility Manager).
1.11. Manifest will contain the following:
- Generators name and mailing address
- Transporter principle name and mailing address
- Phone numbers of each generator and the contractor
- Destination of the facility Name and Address, dated and signed
- Generators Certification and Date
- Medical Waste tracking number
1.12. Provide copies of manifest the day of the pick-up for MacDill and Sabal Park Clinic
1.13. Provide invoice for Sabal Park Clinic to include per container, fuel charge, energy charge, and non-stop charge within 30 days of pick-up.
1.14. Call customer 7 days prior to confirm pick-up dates
2. Service Summary: Pick-up and destroy medical waste for MacDill
and Sabal Park Clinic in accordance with federal, state, and county regulations.
3. Government Furnished Property (GFP) and Services: Government will provide the space and location to set up the containers at the both MacDill and Sabal Park location.
4. Historical Data: MacDill Clinic - Based on current usage, once a month pick-up, between 2000 to 2100 lbs per roll container.
