Modification: FIKE Cheeta Xi Fire Panel Replacement   2

Background
The Indian Health Service, Shiprock Service Unit (SSU), Four Corners Regional Healthcenter (FCHC) located in Red Mesa, AZ 86514 is seeking capable vendors interested in providing the services to satisfy the requirement for Non-Personal Service Contract for replacing FIKE Cheetah XiFire Panel for the Facility Management Department. The Indian Health Services (IHS) is an agency within the U.S. Department of Health and Human Services with a mission to raise the physical, mental, social, and spiritual health of American Indians and Alaska Natives to the highest level.

Work Details
The Contractor shall troubleshoot and repair the Cheetah IT Fire Panel at the Four Corners Regional Healthcare Center. The work includes troubleshooting and repairing the fire panel, coordinating with the Contract Officer regarding necessary parts, not obstructing access and egress of exits during performance, accomplishing all installation and training within ninety (90) consecutive calendar days after official 'Notice to Proceed' is granted by the CO, providing their own tools, labor, materials, supplies, transportation, equipment, supervision, personal protective equipment without smoking or vaping on facility grounds, delivering a written recommended care and maintenance guideline as well as a written _ year warranty on materials and labor on finished product. The Contractor will coordinate with the Facility Manager Tanya Noble-Sudjadi at 928-656-5045 or call the switchboard operator at 928-656-5000.

Period of Performance
The anticipated Period of Performance date for this requirement will be date of award (i.e. 01/18/2024 – 03/31/2024).

Place of Performance
Worksite is at the Four Corners Regional Healthcare Center- Indian Health Service located on Junction U.S. Hwy 160 & Navajo Route 35 - Red Mesa, Arizona.
