Amendment 002. See attached amendment. The proposal due date of 4 March 2022, 11:00 a.m. Central Standard Time remains. The purpose of this amendment is as follows:

1. Post Amendment 2 Attachment 5 - Questions and Answers BACH W81K04-22-R-0012 as of 28 Feb 2022.

2. Provide the following revised document: Revision 1 Exhibit J, W81K04-22-R-0012 WAGE CBA BACH Terra Klean Bassett Army Hospital Supplemental Agreement.

3. Revise Addendum to 52.212-1, subparagraph 2.b.(6)(a)(3) Supplement A-3 Phase-In Plan and subparagraph d(5)(b) Phase-In.

2. Update paragraphs 5.13.3.1. and 5.13.3.4. of the Performance Work Statement;

3. Provide the following revised documents:

a. Revision 1 Attachment 1, MTF Description Workload Data;

b. Revision 1 Attachment 3, Price Matrix;

c. Revision 1 Exhibit D, Detail Building/Rooms/Sq Ft/Type Service;

d. Revision 1 Exhibit G, Snow and Ice Removal,

4. Revise Addendum to 52.212-1, subparagraph (2)(a) to Productive hours proposed by type of services (e.g., Type III, RMW, etc.), labor category (e.g., healthcare environmental cleaning services technician, etc.), and shifts (1st, 2nd, and/or 3rd) for each building. The staffing chart shall also include the total productive hours and the proposed staffing levels based on FTEs.
