Fisher Medical and Lab supplies

Background
The Indian Health Service (IHS) – Phoenix Area Office (PAO) has a requirement for general Fisher medical supplies. The contract will be awarded in a lump sum dollar amount for a base year and 4 option years, with supplies and orders placed against the dollar amount. Delivery will be to multiple locations.

Work Details
Contractor shall provide general clinical laboratory testing supplies for clinical laboratory testing and related support for items that are not available through the Phoenix Area prime vendor contract, in support of patient care at Parker Indian Health Center, Parker, Arizona; Peach Springs Health Center, Peach Springs, Arizona, and the Supai Health Clinic, Supai, Arizona. The reagent supply requirements must be capable of providing listed reagents and supplies on an as-needed basis. Other patient testing supplies needed for clinical laboratory testing that are not specifically listed in this contract may be requested from the contractor on an as-needed basis. All items must ship with the longest dating possible – no short-dated items will be accepted.
All items shipped with single lot number per shipment. Mixed lot numbers will not be accepted.

Period of Performance
Base year is for a period of 12 months effective on date of award. At the discretion of the Indian Health Service, the period of performance may be modified by written notice to exercise an option up to an additional four (4) option years.
