Coagulation Analyzer plus Contractor Maintenance   2

Background
The solicitation is for the provision of preventive maintenance and calibration services for a Coagulation Analyzer at the Ehrling Bergquist Clinic in Bellevue, Nebraska. The contract will cover all costs including parts, labor, and transportation for the equipment. The period of performance for this contract will be the effective date of award and will consist of a Base Year plus four Option Periods of one year each plus a 6-month extension.

Work Details
The contractor is required to provide all personnel, equipment, tools, test equipment, transportation, regularly scheduled preventive maintenance parts, reagents, panels, consumables, and services necessary to perform preventive maintenance and calibration on-site for the Coagulation Analyzer. Additionally, the contractor shall provide on-site instrument training at the time of installation of the analyzer and off-site instrument training for one employee during the first year. The contractor shall also provide a Contract Manager who shall be responsible for the performance of the work.
Quality control and quality assurance measures are specified to ensure compliance with Department of Health Agency (DHA) requirements and manufacturer specifications. The contractor shall furnish all items needed to perform this contract according to all of its terms including specific requirements for the Coagulation Instrument such as utilization of a Prothrombin Time reagent with an International Sensitivity Index (ISI) value of 1.00 or as near to this as possible, bar code reader for patient identification and safety, programmable patient limits and panic values, routine and STAT testing capabilities, on-board tracking of reagent and controls, among others.

Period of Performance
The performance period for this contract will be from the effective date of award and will consist of a Base Year plus four Option Periods of one year each plus a 6-month extension.

The contract work will be performed at Ehrling Bergquist Clinic located at 2501 Capehart Rd, Bellevue Nebraska, 68113.
