This procurement is Buy-Indian set-aside under NAICS code 621310 with a standard business size of $7.5 million. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-04, effective June 2 2023. The Indian Health Services intends to make a single award for Chiropractor Clinics at the Pawnee Indian Health Center in Pawnee, OK.

In support to the information in FAR 52.212-1 & FAR 52.212-2, prospective contractors shall provide the following:
(1) Unique Entity Identifier (UEI) Number (note: Contractors must be registered in the System for Award Management (SAM) database prior to award, during performance and through final payment of any contract resulting from this solicitation. Payments will be made using Electronic Funds Transfer (EFT).
(2) All-inclusive firm fixed-price for Chiropractor Clinics for approximately one year.
(3) An award shall be made to the offeror providing the Lowest Price Technically Acceptable. Evaluation shall proceed as follows:
1. Quotes shall be ranked by price, from low to high.
2. Quotes shall then be evaluated for technical acceptability beginning with lowest priced quote.
3. This process shall proceed until a technical acceptable quote is identified.
It is the responsibility of the offeror to provide sufficient information/documentation demonstrating technical competence. The Government is not responsible for locating or securing any information which is not identified in the offer:
a. Have one (1) year experience as a chiropractor
b. Have current professional liability insurance ($1,000,000/$3,000,000)
c. Have to ability to apply for and be credentialed at Pawnee Indian Health Center
d. Copy of current resume or CV
e. Copy of current license to practice medicine (any US state or territory)
The Government reserves the right to make an award without discussion.
