Campbell County Community Based Outpatient Clinic (CBOC) is to be located in Lafollette, TN.   3

Background
The Government intends to enter into a new real property lease contract for 6,750 rentable square feet (RSF) 5,000 American National Standards Institute/Building Owners and Managers Association (ANSI/BOMA), with a minimum of 25 parking spaces, for utilization as the Campbell County Community Based Outpatient Clinic (CBOC) which is to be located in Lafollette, TN. The tenant agency will be the Veterans Health Administration and approximately 27 full-time Federal employees. The office hours for the facility are proposed to be from 6:00 AM to 6:00 PM, Monday through Friday.

Work Details
The total offer will meet the minimum needs of the specifications. The response should be concise and provide sufficient information to demonstrate the Offeror’s capability to satisfactorily perform the objectives. Plans, written narratives, design concept, renderings, calculations, parking, occupancy timetables, mechanical and electrical systems, energy efficiency of the proposed building are required.
Evidence of compliance with local zoning laws or evidence of variance approved by the proper local authority is necessary.
Proof of Site Ownership or Control is also required. Additionally, various forms such as Solicitation Provisions (Form 3516), General Clauses (Form 3517B), Representations and Certifications (Form 3518), Certification of Building Energy Performance, Small Business Subcontracting Plan, Agency Special Requirements (ASR), Security Requirements, Seismic Form, Prelease Fire Protection and Life Safety Evaluation Form, Davis Bacon Wage Rates Form, Tier 1 Seismic Evaluation Form, Statement of commitment to seismically retrofit the building, Seismic construction schedule and concept design specifications as described in Paragraph 2.03 are all part of the proposal content for standard leases.
