original solicitation. Updated eligible counties table can be found in the "Amendment 1 - Adding Counties" PDF in the attachments section. These counties represent locations where the VA has an active interest in a potential nursing home contract. Only nursing homes located
within the eligible counties list may be considered for a potential contract under this solicitation.

Iowa City Medicaid Solicitation Notice: 36C26324R0010

The Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office (NCO 23), St. Paul, Minnesota is soliciting proposals from Nursing Homes for the Iowa City VA Health Care System to provide community nursing home services under Indefinite Delivery Indefinite Quantity Contracts (IDIQs).

This solicitation is open through January 31, 2025. Proposals will be accepted at any time during this period, up to and including on January 31, 2025. Proposals shall be valid for 180 days from date of receipt. Interested offerors will be responsible for downloading copies of the solicitation (and any subsequent amendments) from that website, and no hard copies will be made available. The solicitation provides instructions to offerors about the proposal submission process. Offerors are encouraged to review the solicitation on the Contract Opportunities website (https://sam.gov/search/and submit their proposal via email to the relevant point of contact as identified in the solicitation as soon as practicable.

In accordance with FAR 52.204-7, System for Award Management (SAM), Nursing Homes must be registered in SAM. Nursing Homes must also register in the VetCert database if claiming VOSB/SDVOSB status under the applicable NAICS. Nursing Homes may obtain SAM information at https://www.sam.gov and VetCert at https://veterans.certify.sba.gov/. To be eligible for award, Nursing Homes must be registered in the SAM database. Registration is free and potential offerors are encouraged to visit the SAM website as no IDIQs can be awarded to any offeror not registered in SAM.

NAICS Code is 623110 (Nursing Care Facilities) with a Small Business Size Standard of $34.0M. The classification code is Q402. The solicitation will be issued for full and open competition. Multiple awards are anticipated and will be made on an as needed basis. See County list in the Instructions to Offerors section of the solicitation. All responsible sources must submit a proposal to be considered by VA.

VA will only consider proposals from offerors who are licensed by their respective States and who are Medicare / Medicaid qualified.

Placements will be made in contract nursing home facilities based on availability of facilities and the desires of the veterans and their respective families.

Background
The Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office (NCO 23), St. Paul, Minnesota is soliciting proposals from Nursing Homes for the Iowa City VA Health Care System to provide community nursing home services under Indefinite Delivery Indefinite Quantity Contracts (IDIQs). The solicitation provides instructions to offerors about the proposal submission process. VA will only consider proposals from offerors who are licensed by their respective States and who are Medicare / Medicaid qualified.

Work Details
The solicitation will be used to award multiple contracts for up to five years. The Community Nursing Home program is a key component of the Veterans Health Administration (VHA) continuum of care. Contractors must furnish all supplies and services at per diem rates for the specified levels of care. Orders may be issued by Ordering Officers to place Veterans in Nursing Homes utilizing the CPRS system at VA hospital sites.
Nursing home facilities in the Contractor program must ensure that care meets the health needs and promotes the maximum well-being of VA patients. They must also maintain a current Center for Medicare and Medicaid Services (CMS) certification and a current State nursing home license. The Contractor will be responsible for providing primary medical coverage, rehabilitation therapy, emergency care, financial responsibility, advanced directives, HIPAA compliance, quality standards, QASP metrics, reportable events, VA staff access to Contractor records, acceptable safety and sanitation practices, leave of absence (LOA) – bed-hold statement, specialty services, and VA Health Care System consultation/resources.
