Custom Dental Packs   2

Background
The Department of Veterans Affairs is seeking to procure custom dental packs, specifically Teeth Cleaning Kits and Denture Cleaning Kits, to support the oral care needs of Veterans. This initiative is part of the VHA HAPPEN program aimed at preventing Non-ventilated Hospital-Associated Pneumonia (NV-HAP) among patients. The contractor will provide all necessary resources to fulfill the requirements outlined in the statement of work (SOW).

Work Details
The contractor shall deliver the following items in sealed perforated plastic bags:

1. **Denture Cleaning Kit** (Quantity: 24 cases per year)
- 1 Denture Adhesive, 2.4 oz
- 1 Denture Brush
- 1 Denture Cup with Lid
- 7 Denture Cleanser Tablets
- 1 Lip Balm, Petroleum Free, SPF 15
- 1 Mouthwash, Alcohol Free, 2 oz.
- 1 Toothpaste Sparkle, .85 oz
- 1 HAPPEN Oral Care Instruction Insert

2. **Teeth Cleaning Kit** (Quantity: 120 cases per year)
- 1 Toothbrush Cover
- 1 Lip Balm, Petroleum Free, SPF 15
- 1 Mouthwash, Alcohol Free, 2 oz.
- 1 Soft Toothbrush, ADA approved
- 1 Toothpaste Sparkle, .85 oz
- 1 HAPPEN Oral Care Instruction Insert

The kits must be delivered monthly on the first Wednesday of each month to the Charlie Norwood VA Medical Center in Augusta, GA. All products must be unopened and free from damage upon delivery.

Period of Performance
Base period from April 1, 2025 to March 31, 2026 with four optional years following.

Place of Performance
Delivery will be made to Charlie Norwood VA Medical Center, Warehouse Building Bldg.111, located at 1 Freedom Way, Augusta GA 30904.
