Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

Solicitation W50S9F-21-Q-0006 is issued as a Request for Quotation (RFQ).

All solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-03, effective 16 February 2021, and Defense Acquisition Circular DPN 20210115, effective 15 January 2021.

This RFQ is solicited as a Total Small Business Set Aside. The anticipated NAICS code is anticipated to be 621340 - Offices of Physical, Occupational and Speech Therapists and Audiologists with a Small Business Size Standard of $8.0M.

The 115th Fighter Wing Operations Group anticipates the following deliverables:

Item 1: Athletic Trainer
Item 2: Virtual Strength and Conditioning Specialist (Includes providing Cervical, Thoracic and Lumbar spinal medical equipment)
Item 3: Athletic Trainer (Option Year)
Item 4: Virtual Strength and Conditioning Specialist (Includes providing Cervical, Thoracic and Lumbar spinal medical equipment) (Option Year)

(vi) Description of requirement:

The 115th Fighter Wing, Air National Guard Base, in Madison, WI has a requirement to provide preventative (pre-habilitative) physical therapy and athletic training services. Surveys indicate up to 100% of fighter aircrew have experienced some type of spine discomfort during their career. This includes issues in the neck, upper back, and lower back (cervical, thoracic, and lumbar spine). This is caused by the dynamic movement pilots experience while operating aircraft under high G forces, and the unique stresses caused by the equipment that is necessary for flight. The helmet that is worn during flight weighs approximately 5 pounds. This weight, combined with experiencing up to 9 G's (9 times the force of gravity) the neck and back can be subject to supporting over 130 pounds in a dynamic maneuvering scenario. Often these forces are exerted while looking over the shoulder while the torso is twisted. The confluence of these factors may result in several types of neck and back injury. The Optimizing the Human Weapon System (OHWS) program is intended to address these pressing concerns and better prepare fighter pilots to meet the unique physical needs of fighter aircraft flying operations. The 115th Fighter Wing has a requirement to provide preventative (pre-habilitative) physical therapy and athletic strength and conditioning services to aircrew at Truax Field Madison, WI. The OHWS program is designed to give fighter pilots the tools and access to professional strength and conditioning coaches, physical therapist and athletic trainers, to prevent future injury and increase quality of life.

Delivery is desired not later than 30 days after contract award. The delivery and acceptance shall be 3110 Mitchell St, Madison, WI 53704. Shipping shall be FOB Destination.

The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Reference solicitation 52.212-2 for evaluation factors.
The following additional contract requirement(s) or terms and conditions determined by the contracting officer to be necessary for this acquisition:

This effort is a high priority requirement in support of the National Guard. Funds are not presently available to make award. If funds become available for award, and prices are deemed fair and reasonable, it is the Governments intent to make award. This is not a commitment of funds or contract award.

DPAS Rating does not apply to this acquisition.

Instructions to Offerors:
1. Quotes must be received no later than the time and date indicated on this RFQ (Time is Central Time Zone). Quotes must be emailed to the primary point of contact shown. It is your responsibility to make sure your quote is received.
2. Quotes must be submitted by using the 1449 CLIN structure.
3. Quotes must include the following
a. Offeror's CAGE Code or DUNS Number.
b. Proposal submission shall contain information with enough detail to facilitate a comparative evaluation in accordance with instructions in 52.212-2 Evaluation-Commercial Items.
4. All contractors shall be fully registered in the System for Award Management (SAM) at the time a quote is submitted to the government. Offerors shall ensure SAM registration is active. The official SAM website is https://www.sam.gov/SAM/.
5. It is the interested Offeror's responsibility to check Contracting Opportunities on beta.sam.gov for updated information.
6. All offeror's will complete and return attached representation for 52.204-24 in addition to the representations/certifications accomplished within SAM.

(xvi) All questions shall be in writing. Questions shall be emailed to the primary point of contact shown on this RFQ. Questions not received within a reasonable time prior to the response date may not be answered.
