Support public and private evidence-based interventions and innovative models to improve the health outcomes of racial and ethnic minority communities. Conduct policy demonstration which coordinate integrated services, community outreach and systems navigation to improve access to care for minorities and disenfranchised communities.

B - Project Grants

Applicants must demonstrate experience in conducting programs in the same focus area which the application addresses. State governments, county or parish governments, city or township governments, special district governments, U.S. Territories, independent school districts, public and state controlled institutions of higher education, Native American tribal governments (federally recognized), public housing authorities/Indian housing authorities, Native American tribal organizations (other than federally recognized tribal governments), nonprofits with or without 501(c)(3) status other than institutions of higher education, and private nonprofit institutions of higher education.

Target populations: American Indian or Alaska Natives; Asians; Blacks or African Americans; Hispanics or Latinos; Native Hawaiians or other Pacific Islanders; economically and/or environmentally disadvantaged; and limited English proficient populations. However, services may not be denied to any otherwise eligible persons..
